Find out how to replace a lost or misplaced vaccination card

WEST PALM BEACH, FLA (August 6, 2021) – If you have lost or misplaced your COVID-19 vaccination card and would like to request a replacement, you will need to contact the same organization you used to get your vaccination.

If you received your vaccination from the Florida Department of Health (DOH), please email: chd50contactus@flhealth.gov with “Vaccination Card Request” in the subject line.

If you received your vaccination from the Health Care District of Palm Beach County, please request your replacement card from their website. www.hcdpbc.org.

If you received your vaccination card at the City of West Palm Beach’s ProtectWPB vaccination site, please request your replacement card from the DOH. Email: chd50contactus@flhealth.gov.
